Request a Participation Kit

Joining the SMGF Project is free, convenient and confidential. Your participation will help the Sorenson Database grow and can help you make new genealogical connections throughout the world and across generations. Participation involves the following simple steps.

  1. Request a Kit
  2. Take a DNA Sample
  3. Prepare Your Pedigree
  4. Read and Sign the Consent Form
  5. Return the Completed Kit

Note: You may request a kit by an e-mail addressed to , but be sure to include all the information found in the form below. You may also request kits for other family members (for instance, wives may order for their husbands), but provide their personal information with your request.

REQUIREMENTS
I am at least 18 years old, or at least 7 years old and have parental consent
I can provide names, birth dates, and birth places for myself and my ancestors
All family lines on my pedigree chart end with ancestors born before 1850 or outside the USA
I understand that I and donating my sample for research and will not receive any results back from SMGF
I have read the consent form and agree to its terms
